This information is recorded directly onto our Practice Management Software, Splose. Records for each attendance are fully completed and saved securely onto this software, which is compliant with the General Data Protection Regulation (GDPR) 2020.

We may also collect your personal information from third parties such as Support Coordinators and Home Care Package providers. We collect your personal information from these third parties with your consent to facilitate clinical care.

We use personal information for purposes in connection with clinical care activities,
including the following purposes:

● Provide you with informed and appropriate health care

● Facilitate coordinated care with other healthcare providers

 Internal administrative purposes
